Our center in Somers offers office-based primary care, plus caregiver support, cognitive therapy, and social wellness programming you won't find bundled anywhere else in Westchester County.

Led by Diane Pagan, ANP-BC

Diane built Senior Moments Care from the ground up in 2023, starting with house calls for older adults who needed more than a rushed office visit could offer. That vision grew into our Geriatric Center of Excellence in Somers, where patients and families can access medical care, caregiver support, cognitive therapy, and community, all in one place. Diane is Hospice & Palliative Care Certified, Care Management Certified, and board certified as an Adult Nurse Practitioner, and her work has been recognized by The Examiner, Westchester Magazine, and Northwell Health.
