Diane Pagan has a Master of Science in Nursing and is board certified as an Adult Health Advanced Practice Provider. She has worked in nursing homes, assisted living facilities, internal medicine, and various house call settings throughout the Bronx and Westchester. She is certified in Hospice and Palliative Care, Medical Cannabis, and Care Management.
Diane realized there is a dearth of healthcare providers today capable of managing the complex needs of our growing older adult population.

For medicare/medicaid eligible patients, and with respect to any services other than the routine exam and geriatric care management services identified above, practice may deliver services specifically covered by applicable plan at patient’s request and as medically indicated and consistent with those plan’s reimbursement requirements.
