Meet Diane Pagan, MSN, ANP-BC, ACHPN, CCM
Diane Pagan has a Master of Science in Nursing and is board certified as an Adult Health Advanced Practice Provider. She has worked in nursing homes, assisted living facilities, internal medicine, and various house call settings throughout the Bronx and Westchester.
She is certified in Hospice and Palliative Care, Medical Cannabis, and Care Management.
Diane realized there is a dearth of healthcare providers today capable of managing the complex needs of our growing older adult population.
Thus, she founded Senior Moments Care, a unique, age-friendly organization which merges primary care with geriatric care management.
What differentiates Diane’s care of older adults from the multitude of conglomerate healthcare organizations in the world today is her empowerment of patients.
Having worked for 25 years within the medical arena, Diane was able to combine her knowledge and experience of what doesn’t work within the healthcare system and created a direct primary care practice based solely upon a single factor…what matters most to each older adult.
